General CME Activity Forms

If you are a Activity Director, CME Activity Committee Member, Moderator, Panelist, Planning Committee Member, or Presenter of a REACH Burn Foundation accredited non-conference activity, you are required to fill out a financial disclosure form so that we can review and identify any potential conflicts of interest.

If you are a presenter of a REACH Burn Foundation accredited non-conference activity, we also need you to review the information and fill out the form on the Content Validation & HIPAA Compliance page.
